We begin by understanding who you really are. Your One Page Profile and support plan identify what matters most to you and are created with you and the people who matter in your life. This active document guides us in helping you achieve your goals and stay connected to the things you love—setting you up for a full and happy life.

Annual Person-Centred Reviews


Every year, we celebrate your achievements through a Person-Centred Review. This process reminds us what matters to you, explores new possibilities, identifies challenges, and agrees on how we can best support you to live the life you choose.

What are Person-Centred thinking tools?

At Discovery, we use person-centred thinking tools—simple, structured templates that guide meaningful conversations. These tools help us capture important information for person-centred planning, improving understanding, communication, and relationships.

Integrated into Your Support Plan
Some of these tools form part of your personalised support plan, ensuring care that truly reflects your needs and goals. We record and share this information through iPlanit, our secure digital platform.

Family Access Made Easy
Families can view support plans, daily records, photos, and calendars via our family portal, keeping everyone informed and connected.

What does a Person-Centred Review look like?

Every Person-Centred Review is unique because it’s all about you. As much as possible, the process is led by you—it’s about you, for you, and done with you.

You can invite anyone important to you to contribute before and during the review. These reviews are creative, engaging, and celebratory, tailored to your gifts and preferred communication style. They usually last around two hours and are scheduled at a time that suits you best.

Our ‘Activate’ model

At Discovery, our approach is always person-centred active support – because every moment in your life matters. We’re here to help you live the life you choose, fully and happily.

Our Activate framework helps us set meaningful goals with you across different areas of life and break them down into achievable steps. Every moment in your day has potential, and Activate ensures we make the most of it.

We use the Activate method alongside the iPlanit app to organise and deliver your person-centred support, keeping everything clear, structured, and accessible.

Video support plans

At Discovery, some people we support choose to have a video support plan alongside their written person-centred plan. Video doesn’t replace the plan—it enhances it.

Why Use Video?
Video helps capture aspects of a person’s personality, preferences, and routines that are hard to describe in words. It ensures colleagues provide consistent support, reducing anxiety and improving quality of care.

Benefits of Video Support

  • Demonstrates how a person responds to the right support
  • Shows preferred routines and communication styles
  • Increases control and independence by making support more accurate
